Current South Carolina Dental Hygiene Licence
Hygienists should send:
- Your current resume.
- A copy of your social security card. .
- Your Alien Registration Receipt Card (green card) if you are a lawful permanent resident.
- Your current driver’s license.
- A copy of your Hepatitis B immunization records from your doctor’s office or health clinic.
- Names and contact numbers of two or three professional references
- Your South Carolina Dental Hygiene Pocket Card
- Proof of Liability Insurance, if you have it. Most dentist offices do not cover temporaries’.
Application requirements for dental assistants
Diploma from a National certified Dental Assisting Program or 2 years of continuous work as a clinical dental assistant
X-ray certification
Assistants should send:
- Your current resume.
